Nhanya Foundation provides Support Coordination for people with disability who need practical, steady support to understand their plan, connect with services, and work towards their goals.
Nhanya Foundation is a not-for-profit organisation and registered charity. Any surplus generated through our services is applied to our charitable purposes and community benefit, including support for women, children, young people and families experiencing family violence, homelessness, financial hardship and other forms of disadvantage. Our service model is intentionally focused, allowing us to provide personalised guidance, connect people with the right services, and deliver high-quality support that responds to the unique needs of the individuals and communities we serve.
Our approach is calm, respectful and focused on the person. We help participants and their support networks make sense of available supports, choose suitable providers, and build confidence to manage services over time.
Support Coordination is not about taking control away from the person. It is about helping them understand their options, make informed decisions, and stay connected with supports that work in real life.
A referral to Nhanya supports two outcomes: focused Support Coordination for the participant, and stronger charitable capacity to support women, children, young people and families through outreach, life-readiness and practical community support.
Our Support Coordinators can help you understand your funding and service options, connect with suitable providers, set up services and referrals, coordinate communication between providers, and prepare for reviews or changes in circumstances.
We can also support with exploring housing, education, employment, therapy, daily living and community participation options. The focus is always on helping the person build a support system that is practical, reliable and suited to their goals.
Nhanya means persistence. That is how we approach Support Coordination.
As a not-for-profit organisation, our focus is on long-term community benefit. We do not provide direct care, SIL, personal care, transport or every support service under one roof. This means there is no internal pressure to direct participants into our own daily support or accommodation services. Our role is to provide transparent Support Coordination that helps participants choose supports that genuinely suit their goals, culture and circumstances.
Nhanya Foundation provides Support Coordination, Specialist Support Coordination, Allied Health Therapy Services and selected capacity-building, life-readiness and community pathway initiatives. We do not provide all support services. Where one of our own services may be relevant, we explain this openly and protect the participant’s right to consider other providers.
We work patiently with participants, families, providers and community services to help reduce confusion, resolve barriers, and keep supports moving. We do not rush people through a process. We take the time to understand what matters, what is not working, and what needs to happen next.
Our Support Coordination is grounded in clear communication, practical problem-solving, respect for choice and control, cultural understanding, and a focus on long-term stability rather than quick fixes.
Support Coordination should protect a participant’s choice — not limit it.
Nhanya Foundation supports participants to choose providers that suit their needs, goals, culture and preferences. Our role is to help people understand options, compare services, ask the right questions, and make informed decisions.
Our Support Coordination is not tied to the use of any particular provider or service. Where any potential conflict of interest could arise, we are transparent and place the participant’s choice and best interests first.
Support Coordination may be suitable if you need help to understand and use your supports, connect with reliable providers, manage multiple services, resolve service gaps, prepare for a review, or build confidence to manage supports more independently over time.
We welcome participants from culturally and linguistically diverse communities and boriginal and Torres Strait Islander peoples, and can consider language, culture, family and community context when coordinating supports.
