Assertive Outreach & Stabilisation Program

This outreach initiative provides practical, relationship-based support for young people aged 12-25 who may be experiencing instability, disengagement, safety concerns or difficulty accessing services. It is designed to build trust, improve routine and connection, and help young people take realistic steps towards greater safety and stability. It welcomes young people from CALD communities and Aboriginal and Torres Strait Islander communities.

Who is it for

  • Young people aged 12–25 who may
  • be experiencing instability or safety concerns
  • feel disengaged from school, work, or community
  • face challenges with emotional wellbeing or behaviour
  • need support accessing services or navigating systems
  • lack structure, routine, or stable support networks
  • be going through significant life transitions

Objectives

  • Build trust and consistent engagement
  • Improve safety awareness and emotional regulation
  • Develop independence in daily life and practical tasks
  • Connect young people with community supports and services
  • Strengthen confidence, decision-making and social skills
  • Support transition into education, training or employment pathways
